Membership Eligibility

To be a member at Blackhawk Community Credit Union, the applicant (or an immediate family member) must reside, work, attend school, or own property in:

Wisconsin

  • Rock County
  • Walworth County
  • Green County
  • Jefferson County
  • Dane County
  • Kenosha County
  • Racine County
  • Waukesha County
  • Milwaukee County

Illinois

  • Winnebago County
  • Boone County
  • Lake County

Account Opening Requirements

Thank you for your interest in making Blackhawk Community Credit Union your financial partner. We look forward to assisting you as you set up your new account. To ensure this is a smooth process, please see the required documentation below to establish your account.
US Citizen Membership:
  • Current Identification (ex. State Issued Photo ID, Passport)
  • Social Security Card
  • Proof of Address-if different than on ID (ex. Utility Bill)
Non-Citizen Resident Alien Membership:

            ITIN Membership
  • Non-expired passport with Visa, if no Visa then an I94 or I797A (Arrival or departure records)
  • ITIN Letter or card from the US Department of Treasury
  • Utility bill or lease agreement verifying address
  • Note: A national IS (Matricular Card) does not meet the qualification for Primary ID requirements)
            Work Only Social Security Number Membership
  • State issued ID, Driver’s License or Government ID
  • Passport with or without Visa or form I797 (DACA)
  • Work only Social Security Card and non-expired work authorization card
  • Proof of address (if not listed on State issued ID, Driver’s License or Government ID)
Undocumented and Documented Immigrants/Temporary Visas:

We are unable to extend membership at this time

All signers are required to provide personally identifiable information (PII) to establish an account. PII includes Name, Address, Date of Birth, Social Security Number and Valid ID showing current address. When the address doesn’t match the ID, additional documents will need to be shown for proof of address.

Specialty Account Opening Requirements

Power of Attorney:

  • Legal Financial Power of Attorney Documentation
  • Photo ID for the Power of Attorney
Trust:
  • Certificate of Trust
  • Copy of SS-4 (if reporting under an EIN)
  • Photo ID for all Trustees
Estate:
  • Domiciliary Letter
  • Copy of SS-4
  • Death Certificate
  • Photo ID for the Personal Representative
Representative Payee:
  • Letter of appointment from SSA or Government issued check made payable to the Rep Payee on behalf of the ward.
  • Photo ID for the Representative Payee
  • Photo ID for the Ward
Guardianship or Conservator:
  • Letters of Guardianship/Conservatorship of the Estate from the court (not Guardianship of the person)
  • Photo ID for the Guardian or Conservator
  • Photo ID for the Ward
Campaign:
  • Certified copy of candidacy paperwork
  • Copy of SS-4
  • Photo ID for Authorized Signer

Business Membership Eligibility

1. Located within our 13-county service area:

For businesses, organizations, or associations to qualify for membership at BHCCU, a majority of the directors, members, or owners must qualify for membership, or the business, organization, or association needs to have a business location within Blackhawk Community Credit Union’s field of membership, as described below.

  • Copy of SS-4 (if using an EIN)
  • Registration of Firm Name (if applicable)

Partnership:

  • Copy of SS-4 or proof of EIN
  • Copy of Partnership Agreement

LLC:

  • Copy of SS-4 or proof of EIN
  • Articles of Organization
  • Operating Agreement

Corporation:

  • Copy of SS-4 or proof of EIN
  • Articles of Incorporation
  • Copy of Bylaws

Organization:

  • Copy of SS-4 or proof of EIN  
  • Documentation authorizing who can open account (ex: meeting minutes)

Church or Non-Profit:

  • Copy of SS-4 or proof of EIN
  • Articles of Incorporation
  • Copy of Bylaws
  • Documentation authorizing who can open account (ex: meeting minutes)
  • Proof of 501 (c)(3) status from the IRS (if applicable)
